Understanding What These Drugs Are Actually Doing

Glucagon-like peptide-1 receptor agonists work by mimicking a naturally occurring hormone that your gut releases after eating. They slow gastric emptying, enhance insulin secretion in a glucose-dependent manner, suppress glucagon, and — critically for weight loss — act on appetite-regulating centers in the brain to reduce hunger and food-seeking behavior. The result is a significant reduction in caloric intake, often without the person feeling deprived in the traditional sense.

Ozempic (semaglutide 0.5 mg to 2 mg, weekly injection) was originally approved for type 2 diabetes management. It produces meaningful weight loss as a secondary benefit, but it was not designed or dosed specifically for obesity treatment. Wegovy is the same molecule — semaglutide — but titrated up to 2.4 mg weekly, a dose specifically engineered to maximize weight loss outcomes. This distinction is one of the most important and most frequently misunderstood points in the entire GLP-1 conversation.

The STEP clinical trial program evaluated subcutaneous semaglutide 2.4 mg across thousands of participants, and the results across STEP 1, 3, 4, and 8 were remarkably consistent. A comprehensive 2023 review in Diabetes, Obesity & Metabolism found that semaglutide 2.4 mg was associated with mean weight losses of 14.9% to 17.4% from baseline at 68 weeks, with 69% to 79% of participants achieving at least 10% weight loss and 51% to 64% achieving at least 15% — compared to just 12% to 27% and 5% to 13% respectively in placebo groups. That is not a marginal effect. That is a pharmacological intervention producing outcomes that rival bariatric surgery in some respects.

The STEP 3 trial added an important variable by pairing semaglutide 2.4 mg with intensive behavioral therapy and an initial low-calorie diet. At 68 weeks, participants on semaglutide lost an estimated 16.0% of baseline body weight compared to 5.7% in the placebo group — a difference of more than 10 percentage points. More than 86% of semaglutide-treated participants achieved at least 5% weight loss, while 55.8% hit the 15% threshold. The takeaway here isn’t just that the drug works — it’s that combining pharmacotherapy with structured behavioral support amplifies outcomes beyond what either achieves alone.

And critically, STEP 4 answered one of the most important clinical questions of all: what happens when you stop? Participants who discontinued semaglutide after a 20-week run-in gained back significant weight — averaging a +6.9% body weight change versus an additional -7.9% in those who continued treatment, representing a 14.8 percentage point difference. Obesity behaves like a chronic disease, and these drugs appear to need to be treated accordingly — as long-term therapy rather than a finite course.

Where Tirzepatide Changes the Equation

Tirzepatide (Mounjaro for diabetes, Zepbound for obesity) represents a meaningful pharmacological step forward. Unlike pure GLP-1 agonists, tirzepatide is a dual agonist — it activates both GLP-1 and GIP (glucose-dependent insulinotropic polypeptide) receptors simultaneously. GIP receptors are found in fat tissue, the brain, and the gut, and their co-activation appears to produce additive metabolic effects that semaglutide alone cannot replicate.

The clinical evidence here is compelling. A 2024 systematic review and network meta-analysis published in Diabetologia, covering 28 randomized controlled trials with over 23,000 participants, found that subcutaneous tirzepatide 15 mg was the most efficacious treatment in reducing both HbA1c and body weight compared to semaglutide at any dose among adults with type 2 diabetes. This is a head-to-head comparison informed by the full body of RCT evidence, not just one trial — and tirzepatide came out ahead.

In the SURMOUNT-1 trial (not yet included in our reference set but widely replicated in the literature), tirzepatide 15 mg produced mean weight reductions of approximately 20-22% in adults with obesity without diabetes — a figure that outpaces even the Wegovy data. For men carrying significant metabolic burden, that delta matters. The difference between losing 15% and 22% of body weight is not academic — it can mean the difference between resolving sleep apnea, normalizing blood pressure, or coming off additional medications entirely.

That said, tirzepatide’s gastrointestinal side effect profile is broadly similar to semaglutide — nausea, vomiting, and diarrhea are the most common complaints, especially during dose escalation. Neither drug is free of trade-offs, and any man considering these medications should understand that tolerability varies individually. Some men sail through titration with minimal discomfort; others find the GI burden significant enough to affect daily function and training capacity.

Oral Semaglutide, Older Agents, and the Evolving Landscape

For men who have an aversion to weekly injections, oral semaglutide (Rybelsus for diabetes, and now the higher-dose Rybelsus formulations being studied for obesity) opens a different door. Historically, oral GLP-1 delivery was challenged by poor bioavailability — the peptide molecule degrades in the stomach before it can be absorbed. Novo Nordisk solved this with SNAC (sodium N-[8-(2-hydroxybenzoyl) amino] caprylate), an absorption enhancer that protects the molecule during gastric transit.

The OASIS 1 trial tested oral semaglutide at 50 mg daily — a substantially higher dose than anything previously used orally — and the results were genuinely surprising. Adults with overweight or obesity without type 2 diabetes achieved an estimated mean body weight reduction of 15.1% over 68 weeks with oral semaglutide 50 mg, compared to 2.4% with placebo — a treatment difference of 12.7 percentage points. More than 85% of participants achieved at least 5% weight loss, and 34% reached the 20% threshold. This brings oral semaglutide into genuine competitive territory with subcutaneous Wegovy, which is a notable development for the field and for patient choice.

Gastrointestinal events were reported in 80% of oral semaglutide participants versus 46% of placebo — a significant gap, and one that’s slightly higher than what’s typically seen with the injectable formulation. The oral route appears to involve more direct GI exposure to the drug, which likely explains the higher incidence of nausea and related symptoms. That’s a real consideration when weighing convenience against tolerability.

Stepping back in time slightly, liraglutide (Saxenda) was the first GLP-1 agonist specifically approved for weight management, requiring daily injections at up to 3 mg. While effective relative to placebo, its weight loss data — typically 5% to 8% mean reduction — looks modest next to the semaglutide and tirzepatide numbers. It also carries a heavier injection burden. For most men starting GLP-1 therapy today, liraglutide is rarely the optimal choice unless tolerability or cost considerations make it the most practical option. Exenatide (Byetta, Bydureon), another older agent, similarly underperforms relative to newer molecules and is largely being phased out of first-line consideration in obesity contexts.

One variable that doesn’t get discussed enough in comparative GLP-1 coverage is muscle preservation. Men on aggressive caloric restriction — whether drug-assisted or not — are at real risk of losing lean mass alongside fat. The research on GLP-1s and body composition is still evolving, but the magnitude of weight loss these drugs produce makes resistance training and adequate protein intake non-negotiable. If you’re on a GLP-1 medication and not prioritizing both, you may be losing weight in a way that degrades your metabolic rate and physical capacity over time. Compounded semaglutide became widely available during shortages, though FDA guidance has tightened around this as brand-name supply has improved. For men navigating cost constraints, working with a prescribing physician to find the most effective dose that insurance will support — or evaluating whether manufacturer savings programs apply — is a necessary part of the conversation.

What This Means For You

The GLP-1 class has fundamentally shifted what’s pharmacologically achievable in obesity medicine, and the evidence hierarchy is becoming clearer. Tirzepatide appears to lead on peak weight loss magnitude. Semaglutide 2.4 mg (Wegovy) has the deepest clinical trial evidence base for obesity specifically. Oral semaglutide at higher doses is emerging as a legitimate needle-free alternative. And older agents like liraglutide, while functional, are being outpaced by newer molecules on nearly every metric that matters to patients.

What the data also makes unambiguous — particularly from STEP 4 — is that stopping these medications typically reverses most of the benefit. These are not short courses. If you’re considering a GLP-1, the conversation with your physician needs to include a frank discussion of long-term commitment, not just a 90-day trial. Pair any pharmacological approach with resistance training, adequate protein, and sustainable nutrition habits, and you’ve built a foundation that works whether you’re on a GLP-1 or not. The medication accelerates the process — the lifestyle determines what you keep.
