When the STEP trials showed semaglutide producing average weight losses of 15% or more of body weight, the headlines celebrated. What the headlines buried was a quieter, more complicated finding: a meaningful portion of that weight loss came from lean mass. Across multiple obesity trials, roughly 25–40% of total weight lost on GLP-1 receptor agonists has been attributed to fat-free mass — including muscle. For a man who spends years building his physique, that number should prompt serious attention, not panic, but a clear-eyed strategy.
Muscle is not just aesthetic. It is metabolically active tissue that governs insulin sensitivity, resting energy expenditure, long-term weight maintenance, and functional capacity as you age. Losing a significant chunk of it while chasing fat loss is a trade you don’t want to make — and fortunately, the research suggests you don’t have to.
Why Semaglutide Puts Muscle at Risk in the First Place
Semaglutide works primarily by suppressing appetite through GLP-1 receptor activation in the hypothalamus and gut, slowing gastric emptying and dramatically reducing caloric intake.
