Because semaglutide significantly reduces appetite and meal size tolerance, meal composition becomes more important than meal frequency. Focus on nutrient-dense, protein-forward meals rather than trying to hit a specific number of meals per day. A smaller plate of grilled salmon with roasted vegetables and a side of quinoa will serve you far better than three large meals full of processed carbohydrates. The evidence suggests that combining aerobic exercise with GLP-1 therapy improves weight loss maintenance compared to either approach alone — but aerobic exercise without resistance training leaves men vulnerable to the sarcopenic outcome that makes weight regain more likely over time. Think of it this way: the medication creates the calorie deficit; resistance training ensures that deficit is coming from fat, not muscle.

For practical application, a three-day-per-week program built around compound movements — squats, deadlifts, rows, presses, and loaded carries — will cover the major muscle groups efficiently and create the anabolic stimulus needed to counteract the catabolic pressure of caloric restriction. Progressive overload, meaning gradually increasing weight or volume over time, is what separates results from just going through the motions. Keep training sessions to 45 to 60 minutes, especially early in your semaglutide journey when energy levels may be lower due to reduced caloric intake.

There’s also emerging evidence that semaglutide’s benefits extend beyond weight and metabolic health. A 2026 study published in Cell Metabolism found that semaglutide demonstrated chondroprotective effects in an osteoarthritis model through a weight loss-independent metabolic mechanism — reprogramming chondrocyte metabolism via the GLP-1R-AMPK-PFKFB3 axis. While the research is early, it adds another dimension to why maintaining joint health through low-impact resistance and mobility work during GLP-1 therapy is a smart long-term play.

The Takeaway

Semaglutide is a genuinely powerful tool for weight loss — the clinical evidence is not ambiguous on that point. But the men who get the best outcomes aren’t the ones who simply inject the medication and wait for results. They’re the ones who treat the drug as a metabolic reset button and use it to build dietary habits and a training foundation that will outlast the prescription. High-protein meals, fiber-rich whole foods, aggressive hydration, and consistent resistance training aren’t optional add-ons to an Ozempic protocol — they are the protocol. The medication suppresses appetite and extends your weight loss runway; what you do inside that window determines what your body actually looks like on the other side.
