The goal during this phase is muscle retention, not performance maximization — and communicating that distinction clearly to clients prevents frustration and dropout.

Here is what most fitness industry conversations about GLP-1 programs get wrong: they frame the medication as the story. It is not. The science that GLP-1 research has surfaced — about lean mass, cardiorespiratory fitness, body composition, and functional outcomes — applies to every man pursuing better health, regardless of whether he is taking anything at all.

A man can lose 30 pounds on a GLP-1 medication and be metabolically worse off if that weight loss was predominantly muscle. For men who have no interest in GLP-1 medications — those who prefer to manage their body composition through nutrition and training alone — this research validates everything rigorous programming has always emphasized. Build muscle. Eat enough protein. Prioritize cardiorespiratory fitness. Measure progress in terms of what your body can do, not just what it weighs. These principles do not change based on what pharmacological tools are or are not in play.

The fitness industry’s integration of GLP-1 programs also surfaces an important conversation about individualization. The same 2025 review calls for patient-centered, individualized approaches that incorporate body composition, functional status, and comorbid conditions. That language should sound familiar to any qualified personal trainer or sports nutritionist — it is the language of good coaching. The best fitness professionals have always assessed the individual in front of them and designed accordingly. GLP-1 programs simply add a new variable to that assessment, not a new philosophy.

Men who are combining GLP-1 therapy with structured training should track more than their weight. Circumference measurements, strength benchmarks, and ideally periodic DEXA scans or bioelectrical impedance assessments give a clearer picture of whether the weight being lost is predominantly fat or disturbingly mixed with muscle. Fitness apps that only log weight are insufficient for this population. The data that matters is compositional.

There is also an access and equity dimension worth naming. The Alebna review specifically cites cost, insurance restrictions, and provider hesitation as barriers to GLP-1 implementation. For the majority of men who either cannot access or choose not to pursue these medications, the fitness industry’s job does not change — it is to provide evidence-based programming that drives fat loss, preserves or builds muscle, and improves metabolic health through the foundational tools that have always worked: progressive resistance training, nutritional strategy, and sustainable lifestyle change. GLP-1 medications are a legitimate and increasingly important clinical tool. They are not a replacement for the work.

What This Means for You

Whether you are currently on a GLP-1 medication, considering one, or building your physique the old-fashioned way through the weight room and a disciplined plate, the science converging around these drugs carries a message worth internalizing: how much you weigh matters far less than what your weight is made of and what your body can do with it. Cardiorespiratory fitness, muscular strength, and lean mass are the currencies of long-term metabolic health. Any program — pharmaceutical, nutritional, or training-based — that moves those numbers in the right direction is doing its job. Any program that moves the scale without moving those numbers is telling you an incomplete story.

The fitness industry’s engagement with GLP-1 programs is, at its best, an opportunity to deliver more complete care — pairing the metabolic power of these medications with the structural work that only resistance training and intelligent nutrition can provide. At its worst, it is a marketing opportunity dressed in clinical language. The difference lies in whether programs are built around body composition, functional outcomes, and long-term sustainability — or around the number on the scale and the speed with which the medication gets there.

For men who are serious about their health, the standard does not change based on what tools they are using. Train with intention. Eat enough protein. Protect your muscle. Measure what actually matters. That is the program — with or without the injection.
