When Ozempic became a cultural phenomenon, it brought an entire class of medications into the spotlight. But semaglutide — the active compound in Ozempic — is just one player in a growing field of GLP-1 receptor agonists, each with distinct mechanisms, dosing schedules, and clinical outcomes. If you’re trying to make an informed decision about which medication fits your health goals, the differences matter more than most people realize.

GLP-1 (glucagon-like peptide-1) receptor agonists work by mimicking a natural gut hormone that signals fullness, slows gastric emptying, and modulates insulin secretion. The class includes semaglutide (Ozempic, Wegovy, Rybelsus), liraglutide (Victoza, Saxenda), dulaglutide (Trulicity), exenatide (Byetta, Bydureon), and tirzepatide (Mounjaro, Zepbound) — which is technically a dual GIP/GLP-1 agonist. They share a common mechanism but diverge significantly in efficacy, tolerability, dosing frequency, and cost.

Semaglutide: The Clinical Benchmark

Semaglutide has become the reference point against which all other GLP-1 agents are measured, and the data justifies that position. The STEP clinical trial program, which evaluated once-weekly subcutaneous semaglutide at 2.4 mg across multiple populations, produced results that reshaped expectations for pharmacological weight management. Across STEP 1, 3, 4, and 8, semaglutide 2.4 mg was associated with mean weight losses of 14.9% to 17.4% in individuals with overweight or obesity without type 2 diabetes — numbers that approach what many had previously reserved for surgical intervention.

What makes semaglutide’s profile particularly compelling isn’t just the headline weight loss figure — it’s the breadth of benefit. In the STEP 3 trial, participants combining semaglutide with intensive behavioral therapy and an initial low-calorie diet saw mean body weight reductions of 16%, compared to 5.7% with placebo. Over 55% of semaglutide participants achieved at least 15% weight loss, a threshold associated with meaningful improvements in cardiometabolic risk. Those results don’t happen with diet alone, and they don’t happen with older GLP-1 agents either.

Continuity also matters. The STEP 4 withdrawal trial illustrated a critical point about semaglutide’s mechanism: the drug is doing ongoing metabolic work that stops when you stop taking it. Participants who continued semaglutide after an initial 20-week run-in lost an additional 7.9% of body weight, while those switched to placebo regained 6.9% — a 14.8 percentage point divergence. Waist circumference, systolic blood pressure, and physical functioning all continued improving in the semaglutide group. This isn’t a medication you cycle on and off casually; it’s a long-term metabolic intervention.

Oral semaglutide — marketed as Rybelsus for diabetes and now studied at a 50 mg dose for obesity — adds another dimension to the conversation. The OASIS 1 trial found that oral semaglutide 50 mg once daily produced a mean bodyweight reduction of 15.1% over 68 weeks, compared to 2.4% with placebo. For men who are needle-averse or want more flexibility in their regimen, oral semaglutide at therapeutic doses is a legitimate option — though the bioavailability constraints of oral delivery mean strict administration requirements (taken on an empty stomach, with a small amount of water, no food for 30 minutes afterward).

How the Other GLP-1 Agents Actually Compare

Liraglutide was the first GLP-1 agent approved specifically for obesity management, under the brand name Saxenda at a 3 mg daily injectable dose. It works through the same receptor as semaglutide but has a shorter half-life, requiring daily injections rather than weekly. Clinical trials showed mean weight losses of around 5–8% over 56 weeks — meaningful, but substantially less than what semaglutide achieves. For men who started on liraglutide years ago and are evaluating whether to switch, the efficacy gap is real and supported by head-to-head data.

Dulaglutide (Trulicity) is a once-weekly injectable primarily indicated for type 2 diabetes management, not obesity. While it produces modest weight loss as a secondary benefit, it was never engineered or dosed for aggressive fat reduction. Exenatide in its extended-release form (Bydureon) follows a similar pattern — weekly dosing, diabetes-first indication, and weight loss that trails behind semaglutide in both magnitude and consistency.

Tirzepatide deserves special attention because it operates on two receptors simultaneously — GIP (glucose-dependent insulinotropic polypeptide) and GLP-1. This dual agonism appears to produce additive effects on satiety and metabolic regulation. The SURMOUNT trials demonstrated weight losses exceeding 20% of body weight at the highest tirzepatide doses, placing it ahead of semaglutide in raw efficacy terms. For the highest-BMI patients who have struggled with other pharmacotherapy, tirzepatide represents the current ceiling of non-surgical weight loss medication — though its long-term cardiovascular outcome data is still accumulating compared to semaglutide’s more mature evidence base.

That surgical comparison is worth addressing directly. A large 2025 cohort study using real-world insurance and medical record data found that bariatric surgery produced greater total weight loss (28.3%) compared to GLP-1 receptor agonists (10.3%) over two years, while also carrying lower ongoing costs once the surgical episode was complete. For men with class II or III obesity facing serious metabolic consequences, this data reframes the conversation — GLP-1 medications are powerful, but they exist on a spectrum of intervention intensity, not at the top of it.

Choosing between these agents isn’t purely a matter of picking the one with the highest weight loss number. Tolerability shapes real-world outcomes significantly. Gastrointestinal side effects — nausea, vomiting, constipation — are common across the entire class and tend to be more pronounced at higher doses. In the STEP 3 trial, 82.8% of semaglutide participants reported GI adverse events, compared to 63.2% on placebo. Most are mild to moderate and peak during dose escalation, but for some men they’re genuinely disruptive. Starting low and titrating slowly isn’t optional — it’s the strategy that separates men who succeed on these medications from those who quit in the first six weeks.

Muscle preservation is another consideration that deserves more attention than it typically gets in GLP-1 discussions. Rapid weight loss from any source — caloric restriction, medication, or surgery — comes with the risk of lean mass loss. Men on GLP-1 agents who aren’t actively resistance training and consuming adequate protein are likely losing muscle alongside fat. The medication doesn’t protect against this. Pairing any GLP-1 therapy with a structured strength training program and protein intake of at least 0.7–1 gram per pound of bodyweight is non-negotiable if maintaining metabolic rate and physical performance matters to you — and it should.

What This Means For You

The GLP-1 landscape has evolved from a single diabetes drug into a tiered system of metabolic interventions with genuinely different risk-benefit profiles. Semaglutide remains the best-studied option with the strongest long-term data, particularly for men who want robust weight loss combined with cardiovascular benefit signals. Tirzepatide edges it out on raw efficacy for those who need maximum intervention. Liraglutide and dulaglutide are older tools with narrower use cases. Oral semaglutide adds convenience without sacrificing much efficacy at the right dose. And for severe obesity, bariatric surgery still produces greater and more durable weight loss at lower long-term cost than any medication currently available.

None of this replaces the fundamentals. GLP-1 medications work best when they’re supporting a man who is already committed to eating well, training consistently, and prioritizing sleep and stress management. The drug reduces appetite — it doesn’t build the habits, protect the muscle, or create the metabolic resilience that comes from years of deliberate effort. Use the evidence to make a smart choice about which tool fits your situation, then build the rest of the structure around it.
