GLP-1 receptor agonists like semaglutide and tirzepatide have reshaped the conversation around obesity and metabolic health. The 2023 Global Burden of Disease Study, published in The Lancet, identified high BMI as one of only three leading risk factors whose age-standardized burden actually increased between 2010 and 2023 — rising 10.5% — and explicitly named GLP-1 receptor agonists as part of the policy response needed to reverse that trend. That’s a remarkable endorsement from the world’s most comprehensive disease surveillance framework. But the medication is only half the equation. What you eat and how you train while on these drugs will determine whether you lose fat and keep muscle — or simply get smaller and weaker.
Here’s what the science says about building a diet and exercise plan that actually works alongside GLP-1 therapy.
Nutrition: Eat Less, But Eat Smarter
GLP-1 medications suppress appetite significantly. That’s the mechanism — they slow gastric emptying, increase satiety signals, and reduce food-seeking behavior. The result is that most men naturally eat far fewer calories without trying. This is effective for fat loss, but it creates a real nutritional hazard: when total food intake drops dramatically, so does protein intake, micronutrient density, and the raw material your body needs to preserve lean tissue.
The priority on a GLP-1 plan is protein, and it needs to be non-negotiable. Research consistently supports a target of 1.6 to 2.2 grams of protein per kilogram of body weight per day to preserve muscle during a caloric deficit. On a medication that may cut your daily intake to 1,200–1,500 calories, hitting that number requires intentional food selection. Lean animal proteins — chicken breast, eggs, Greek yogurt, cottage cheese, fish, lean beef — should anchor every meal. If appetite suppression makes eating a full meal difficult, a high-quality whey or casein protein shake becomes a practical tool, not an afterthought.
Beyond protein, the reduced food volume means every calorie has to carry nutritional weight. Processed foods, refined carbohydrates, and alcohol all become expensive choices when your appetite budget is limited. Prioritize whole foods: vegetables for fiber and micronutrients, complex carbohydrates like oats and sweet potatoes for sustained energy around training, and healthy fats from sources like olive oil, avocado, and fatty fish to support hormone production and joint health. Eating slowly matters more than ever — the drug already slows gastric emptying, and eating too fast can cause significant nausea and discomfort.
Hydration and electrolytes also deserve attention. Reduced food intake often means reduced sodium, potassium, and magnesium consumption, which can contribute to fatigue and muscle cramping. Aim for at least 2.5–3 liters of water daily and consider a mineral supplement if training intensity remains high.
Training: Resistance Work Is Non-Negotiable
This is where most GLP-1 users leave results on the table. The medication creates a caloric deficit that drives weight loss — but without a strong resistance training stimulus, a significant portion of that weight loss comes from muscle, not fat. Studies on aggressive caloric restriction consistently show that without resistance training, lean mass losses can account for 25–40% of total weight lost. On a GLP-1 medication, where weight loss can be rapid and substantial, that risk is amplified.
Three to four days of resistance training per week is the foundation. Compound movements — squats, deadlifts, rows, presses, lunges — recruit the most muscle mass and deliver the strongest anabolic signal. Progressive overload, meaning gradually increasing the weight or reps over time, is what tells the body that muscle is worth keeping even in a caloric deficit. This doesn’t require a complex program. A straightforward full-body or upper/lower split, performed consistently, will preserve and even build lean tissue while the medication handles the fat loss side of the equation.
Cardio has its place, but it should be used to support cardiovascular health and metabolic conditioning — not as the primary fat-loss tool, since the medication is already doing that job. Two to three sessions of moderate-intensity cardio (30–45 minutes of walking, cycling, or rowing) per week is enough to support heart health without adding excessive recovery demand on top of resistance training. If energy is low — which is common early in GLP-1 therapy — protect the resistance sessions first and scale back cardio before anything else.
The Takeaway
GLP-1 medications are a powerful metabolic tool, and the global disease burden data makes clear they have a legitimate role in addressing the obesity crisis. But a drug that suppresses appetite doesn’t automatically produce a healthy body composition. The men who get the best outcomes — more fat lost, more muscle preserved, better long-term metabolic health — are the ones who treat the medication as an accelerant for smart habits, not a replacement for them. Prioritize protein at every meal, train with weights consistently, and let the medication do what it was designed to do.
