When semaglutide burst onto the weight-loss scene, the headline numbers were hard to argue with — clinical trials showed participants losing 15% or more of their body weight. But buried in that success story is a concern that every man on the medication should understand: a meaningful portion of that weight loss can come from muscle, not just fat. Studies suggest that without deliberate countermeasures, lean mass can account for 25–40% of total weight lost during GLP-1-driven caloric restriction. That’s not a rounding error. That’s the difference between emerging leaner and stronger versus lighter but metabolically weaker.

Semaglutide effectively kneecaps all three pathways simultaneously. Appetite suppression makes hitting protein targets feel like a chore. Reduced caloric intake creates an energy deficit that, without the right stimulus, signals the body to downsize both fat and lean tissue. And many men on the medication report fatigue and nausea early on, which makes the gym feel like a secondary priority.
The stakes are higher than aesthetics. Skeletal muscle is the primary site of glucose disposal in the body. Losing it accelerates insulin resistance, slows your resting metabolic rate, and makes future weight management exponentially harder. Creatine monohydrate is one supplement with enough evidence behind it to earn a mention in any serious muscle preservation conversation. It has been studied extensively in contexts of caloric restriction and aging, and the data consistently show it helps maintain strength output and lean mass. Three to five grams daily is the standard protocol — no loading phase required. It won’t override poor nutrition or a sedentary lifestyle, but combined with training and adequate protein, it’s a legitimate tool.
Finally, pace matters. Semaglutide can drive rapid weight loss, and faster loss correlates with greater lean mass sacrifice. If your rate of loss exceeds one to 1.5 pounds per week consistently, it’s worth discussing a dose adjustment with your prescriber, or intentionally eating more on training days to slow the deficit without abandoning progress. Fat loss that outpaces your ability to protect muscle is a pyrrhic victory.

Semaglutide is a powerful metabolic tool — but it doesn’t know the difference between fat and muscle. That distinction is your job. Hit your protein targets aggressively, train with resistance and intent, prioritize sleep, and consider creatine as a low-cost, high-evidence addition.
