Semaglutide vs. Tirzepatide: What the Latest Science Actually Says

For years, semaglutide was the undisputed heavyweight of obesity medicine. Then tirzepatide arrived, and the conversation shifted entirely. Now, in 2025, we finally have a head-to-head clinical trial answering the question everyone was asking — and the results are more nuanced than either camp expected.

The SURMOUNT-5 trial, published in the New England Journal of Medicine, is the most definitive comparison yet between these two medications in people with obesity but without type 2 diabetes. Over 72 weeks, tirzepatide produced an average body weight reduction of 20.2%, while semaglutide produced 13.7% — a statistically significant difference that translates into a meaningful gap in the real world. Waist circumference dropped 18.4 cm with tirzepatide versus 13.0 cm with semaglutide. Participants on tirzepatide were also consistently more likely to hit the 10%, 15%, 20%, and 25% weight loss thresholds. By the numbers, tirzepatide wins the fat loss head-to-head. Where Semaglutide May Have the Edge: Cardiovascular Outcomes

Here is where the picture gets complicated, and where men focused on long-term health need to pay close attention. Weight loss is not the only metric that matters. Cardiovascular disease remains the leading cause of death in men, and recent data suggests the two medications may diverge when it comes to heart outcomes — in ways that could flip the script.

The STEER study, published in Diabetes, Obesity & Metabolism, analyzed real-world data from over 10,000 matched patients per group — all with overweight or obesity, established atherosclerotic cardiovascular disease, and no diabetes. Semaglutide was associated with a 29% lower risk of a composite cardiovascular endpoint (heart attack, stroke, all-cause mortality) compared to tirzepatide, and a 22% lower risk of a broader five-point MACE endpoint. In the per-protocol analysis, semaglutide’s advantage was even more pronounced — a 57% relative reduction in three-point MACE. These findings suggest that for men who already have cardiovascular disease, semaglutide may carry unique protective effects beyond what weight loss alone can explain.

When it comes to heart failure specifically, both medications show impressive results compared to no treatment. A 2025 observational study published in JAMA evaluated patients with heart failure with preserved ejection fraction — a particularly difficult-to-treat condition closely tied to obesity and metabolic dysfunction. Both semaglutide and tirzepatide reduced the combined risk of hospitalization for heart failure or all-cause mortality by more than 40% compared to a placebo proxy. However, in the head-to-head comparison within that study, tirzepatide showed no meaningful additional benefit over semaglutide for this specific population. Studies consistently show that roughly 25–40% of weight lost on GLP-1-based therapies can come from lean mass when lifestyle factors are not addressed. Men who prioritize body composition — not just scale weight — need to treat these medications as a metabolic accelerator layered on top of a strength training program and a high-protein diet, not as a replacement for either. Hitting 0.7–1.0 grams of protein per pound of bodyweight daily and resistance training three or more times per week are non-negotiable if preserving muscle during significant weight loss is a priority.

For men managing type 2 diabetes who want aggressive HbA1c reduction and maximum weight loss, tirzepatide is the more potent pharmacological choice based on current evidence. For men who are primarily focused on reducing cardiovascular mortality, particularly those with established heart disease, semaglutide’s long-term cardiovascular outcome data is currently more robust. For those managing heart failure with preserved ejection fraction, either medication appears meaningfully beneficial over current alternatives.

The Takeaway

The arrival of a legitimate head-to-head trial in 2025 settled one debate and opened others. Tirzepatide is the more powerful weight loss agent. Semaglutide may hold a cardiovascular advantage in high-risk patients. Both are transformative tools compared to what existed a decade ago, and both dwarf placebo when it comes to reducing the downstream consequences of obesity-related metabolic disease. The right choice depends on your specific health profile, your goals, and an honest conversation with a physician who knows your full picture. What neither medication replaces is the training, the nutrition, and the daily choices that determine whether that weight stays off and whether the muscle you build in the process actually lasts.
